TERMS OF AGREEMENT

-SHORT TERM HIRE OF DEFIBRILLATOR-

1. INSURANCE:

The lessee will have insurance to cover the cost of the unit(s) in case of loss or damage, or if the unit(s) cannot be returned to The Defib Shop Pty Ltd in good working order, they were received. If a defibrillator unit is damaged, lost or stolen or cannot be returned to The Defib Shop Pty Ltd in good order, the lessee agrees to pay for damages or replacement up to $3,795.00 per unit. The Defib Shop Pty Ltd will determine the amount to be paid.

 

2. GST:

GST applies to all transactions for leasing agreements.

 

3. VALIDITY TERMS:

Once full payment has been received, this agreement is signed, and the unit is delivered, the lease will commence. If an extension is required, the lessee will notify The Defib Shop Pty Ltd as early as possible. There are no penalties or extra charges above standard lease charges if reasonable notice is given.

 

4. OPERATION OF UNIT:

The lessee will ensure that only currently qualified and authorised staff(s) or volunteer(s) use the Defibrillator unit. The Defib Shop Pty Ltd accepts no responsibility for anyone using the unit. The lessee is fully responsible for ensuring any units are used in a safe workplace environment.

 

5. CONSUMABLES:

The unit(s) supplied come with a basic set of consumables, including an adult defibrillation pad, a set of batteries, and a patient preparation kit. Should the provided consumables be used or otherwise depleted, the lessee must purchase and replenish the stock of the required items. Consumables are available from The Defib Shop Pty Ltd.

 

6. PAYMENT OPTIONS:

Credit Card Payment:

Paid – accompanying receipt
VISA, MasterCard or AMEX – all have a 2% fee on the total amount.

 

7. SHIPPING:

Shipping is included – delivered by The Defib Shop Pty Ltd couriers – Shipping via regular courier - Road Freight will be arranged by The Defib Shop Pty Ltd. The freight cost to the lessee's address will be charged to the invoice. 

 

8. RETURN SHIPPING:

The return shipping is to be arranged and paid for by the lessee. Any delays in the return of the unit will be charged accordingly.

, and agree to fulfil the terms of this lease with The Defib Shop Pty Ltd.

and I declare I am in the position to authorise the above agreement.

 

I understand this agreement will become effective once the above terms have been agreed to and The Defib Shop Pty Ltd has received the payment as per the above invoice (unless otherwise arranged).

 

INSURANCE:

As per the details above in point 1, the lessee will provide adequate insurance to cover the cost of the defibrillator and the case it is housed in the event of damage, loss, or theft. The unit is to be returned to The Defib Shop Pty Ltd in good working order as it has been received.

 

I understand that if the unit is damaged, lost or stolen, I will pay up to $3,795.00 as determined by The Defib Shop Pty Ltd to replace the unit.
